In consideration of your acceptance of my entry, I do hereby, for myself, my heirs, executors, and administrators waive, release and forever discharge any and all rights and claims for damages which I may have or may accrue to me against MOUNTAIN TAE KWON DO /MOUNTAIN JUDO,(The School) and its INSTRUCTORS, The United States Tae Kwon Do, the United States Olympic Committee, the World Taekwondo Federation, the United States Judo Federation, and all members of, or their respective officers, committees, medical committee, agents, representatives, successors, and/or assignees and against any other member for any and all damages which may be sustained by me in connection with my association with or entry in the above activity, or which may arise out of traveling to, participating in, and returning from this activity. I understand that Tae kwon do, Hapkido, Judo, and Weight Training are a body-contact sport, which involves a risk of injury. I understand all the contents of The School rules and general information published by Mountain Tae Kwon Do/MOUNTAIN JUDO, and I agree with them in their entirety. I understand that if I fail to abide by the rules of Mountain Taekwondo,/MOUNTAIN JUDO, or if my conduct is not cooperative for the successful operation of The School that I may be dismissed from the premises without refund, and my privaleges may be taken away along with all rights and privileges provided by my membership. If I fail to show up for lessons at the designated times published by The School, I knowingly forfeit all applicable membership fees. I understand that I must pay full tuition on the first of each month, or when otherwise designated by contract, if any,and even if I attend only one class for the month. I understand that I will pay all additional charges as a result of collection proceedings if necessary. I understand that refunds will not be given for any reason. I understand that personal and business checks are acceptable forms of payment for registration membership and will pay all late payment fees and or bad check fees related to the same. I further understand that any pictures taken of me in connection with The School may be used by Mountain Tae Kwon Do/MOUNTAIN JUDO, for publicity or promotion without compensation. I hereby agree to all the terms and conditions in the liability waiver above.
